Janssen CarePath Savings Program for XARELTO®

Eligible patients using commercial or private insurance can save on out-of-pocket costs for XARELTO®. Depending on the health insurance plan, savings may apply toward co-pay, co-insurance, or deductible. Eligible commercial patients pay $10 for each 30-day to 90-day prescription for XARELTO®, subject to program benefit limits. There is no limit to this benefit for the first 90 days, and then a $200 limit for each 30-day supply thereafter. There is a $3,400 maximum program benefit per calendar year. Not valid for patients using Medicare, Medicaid, or other government-funded programs to pay for their medications. Patients prescribed XARELTO® 10 mg because of a recent non-surgical hospital discharge or because they have recently undergone hip or knee replacement surgery are not eligible. Terms expire at the end of each calendar year and may change. There is no income requirement.

We provide cost support directly to patients through the Janssen CarePath Savings Program. This benefit is intended to help eligible patients afford their out-of-pocket obligations as set by their health plans. The cost support is meant solely for patients—not health plans and/or their partners. If you are having any difficulty accessing cost support through the Janssen CarePath Savings Program, please contact us at 888-XARELTO (888-927-3586) .

Xarelto assistance for Medicare patients

Lindsay Malzone writer headshot

Xarelto is a blood thinner that’s used to treat or prevent blood clots by blocking the activity of certain clotting substances. It’s commonly known to lower your risk of a pulmonary embolism (PE) or deep vein thrombosis (DVT) coming back after receiving treatment for at least 6 months.

People who suffer from atrial fibrillation (a heart rhythm disorder) can use Xarelto to lower the risk of stroke caused by blood clots. Some doctors may administer Xarelto to patients who are hospitalized for an acute illness if there’s a chance they could get a blood clot.

RELATED: Xarelto side effects and how to avoid them

Does Medicare cover Xarelto?

Yes. In order to get Medicare coverage for Xarelto, you must have a Medicare Part D plan or Medicare Advantage plan with drug coverage. Neither Original Medicare (Part A and Part B) nor Medigap plans cover prescription drugs.

Not every Medicare Advantage or Part D plan will cover a prescription of Xarelto. It’s important to check the health plan’s formulary before enrolling to ensure all your medications are covered.

How much is Xarelto with Medicare?

Without insurance, the cost of Xarelto is around $650 for 30, 20 mg tablets. With Medicare, as well as any insurance, the cost depends on many factors such as the dosage you’re prescribed, the plan you enrolled in, the copay, coinsurance, and the pharmacy you purchase the medication from. Additionally, the costs will vary for people with Extra Help and depending on which coverage phase you are in. Fortunately, Xarelto coupons for Medicare beneficiaries and people without health insurance plans are available to help reduce the cost of the medication.

The prices shown below may vary and do not reflect the restrictions that many Part D plans have for higher tier drugs such as quantity limits, prior authorization, and step therapy.

Xarelto costs with Medicare Part D

Below we’ll compare Part D plans’ out of pocket costs with a few different pharmacy options for Xarelto. The following estimates are from the Plan Finder tool available on medicare.gov .

Typically, you can find a generic version of a brand-name drug which help lower the cost of prescription drugs, but there is no generic alternative to Xarelto available in the United States.

According to Leann Poston , MD, medical expert for invigormedical.com, “As of now, there is no generic version of Xarelto available in the U.S. The generic rivaroxaban is available in other countries but is still long-anticipated in the U.S.”

This is because the patent protection for the active ingredient found in Xarelto (rivaroxaban) was recently extended until August 2024 in the United States. This patent has already expired in other countries such as the United Kingdom, India, and Canada.

SingleCare coupons

The good news is that there is discount pharmacy coupons for Xarelto available through SingleCare. These coupons can save you money, even if you currently have Medicare or another type of insurance.

As stated above, the cost for Xarelto without insurance is around $650 for 30, 20 mg tablets. However, with SingleCare pharmacy coupons, you’ll only pay $402 for 30, 20 mg tablets.

There are situations where you can save more money by using SingleCare instead of your Medicare prescription drug plan. Many Medicare beneficiaries find that SingleCare prices are lower than their out-of-pocket medication costs with Medicare.

RELATED: Can I use SingleCare if I’m on Medicare?

Medicare Extra Help

If you meet the low-income requirements or are dual-eligible for both Medicare and Medicaid, then you can get help paying for Xarelto on Medicare.

The Medicare Extra Help program will reduce your out-of-pocket costs for prescriptions drugs, eliminate your monthly Part D premium, and even eliminate or reduce your Part D annual deductible.

Some beneficiaries fall into the coverage gap, also known as the donut hole. During the donut hole, the beneficiary has a window where their prescription drug insurance coverage is limited.

While in the donut hole , you’ll pay 25% of the cost of generic and brand name drugs until you reach $7,050. Once you reach this amount, you’ll fall into the catastrophic phase of Part D where you’ll only pay 5% of your drug costs.

If this is something you’re concerned with, you can find peace of mind knowing Extra Help will protect you from the coverage gap. This means you won’t pay more for your prescription medicine if your total drug costs exceed $4,430.

Below we’ll compare Part D plans out-of-pocket costs with a few different pharmacy options for Xarelto with have Extra Help

Xarelto coupons may be available through the manufacturer. Contact Janssen Pharmaceuticals, Inc for additional information about their offerings, such as Janssen Select, and to find out if you qualify for financial assistance.

Patient assistance programs

The manufacturer does offer a patient assistance program. Eligible patients using commercial insurance or private insurance plans can save on out-of-pocket costs for Xarelto. This also includes plans from the health insurance marketplace.

The Janssen CarePath Savings Program has no limit for the first 90 days. After 90 days, there’s a $200 limit for each 30-day supply. The maximum benefit per calendar year is $3,400.

Free samples at your doctor’s office

Sometimes the drug company representatives will leave sample packs of the prescription medication at your healthcare professional’s office. However, this is not a long-term solution to save money on your prescriptions. Contact your healthcare provider’s office to see if they have free Xarelto samples available.

Xarelto alternatives

There are a few anticoagulant drugs that can be used in place of Xarelto . The most common include Eliquis , warfarin , Pradaxa , Plavix , and Brilinta . Other less common anticoagulant drugs include Angiomax (bivalirudin), argatroban , Bevyxxa (betrixaban), Effient (prasugrel) , Iprivask (desirudin), Savaysa (edoxaban) , and unfractionated heparin.

There are also a few natural alternatives to Xarelto that can help reduce the risk of blood clots and cardiovascular disease. These natural blood thinners include garlic, turmeric, ginger, vitamin E, and omega-3 fatty acids.

Make sure to always consult with your physician before switching to any Xarelto alternative. RELATED: Eliquis vs. Xarelto: Differences, similarities, and which is better for you

What if my Medicare prescription drug plan doesn’t cover my medications?

If your Medicare prescription drug plan doesn’t cover your medications, there are a few enrollment windows to make changes.

If you have prescription drug coverage through a Medicare Advantage plan, you can switch plans two times during the year:

If you have a Part D prescription drug plan, the only annual enrollment window you can make changes to is during the Annual Enrollment Period in the fall.

You may also be able to switch plans outside of one of these enrollment windows if you had a change in circumstance and met the eligibility requirements for a Medicare Special Enrollment Period.
